The engine coolant temperature sensor is a negative temperature coefficient thermistor in the coolant flange, supplied with a 5V reference through a pull up resistor inside the Engine Control Module, so the signal voltage falls as coolant temperature rises. The module knows the signal can never legitimately exceed its 5V reference, so any voltage above that range identifies an external source feeding the circuit.
